This isn't much of a bug nor a major issue but an easily fixable miss with the ADB activation method on the README.md file. But since I had downloaded the app on multiple devices already I've always had an issue with this mini-point.
The last part of the app's name is lost in the activation command
./adb shell pm grant ru.karasevm.privatednstoggle android.permission.WRITE_SECURE_SETTINGS
Should be
./adb shell pm grant ru.karasevm.privatednstoggle.dev android.permission.WRITE_SECURE_SETTINGS
